New Patients Welcome
We are accepting new patients daily! Please call to schedule your first appointment.
What to Bring:
- Insurance card
- Photo identification
- Current medication list
- Medical records
Cancellation Policy
We understand that life happens and things come up. We contact patients 48 hours in advance to confirm appointments.
There is a standard cancellation fee of $75.00 if an appointment is not cancelled within 24 hours of the original time.
Age Requirements
Heart and Vascular Care currently treats patients 18 years or older
For pediatric cardiovascular care, we can provide referrals to specialists who focus on younger patients.
