Referral Documents
Bring any referral letters or documents from your primary care physician or other healthcare providers.
Current Medications
A complete list of all medications, supplements, and vitamins you're currently taking, including dosages. Most patients bring a bag full of medications they are taking.
Insurance Cards
Your current insurance card and a valid photo ID for verification and registration purposes.
Previous Test Results
Any recent cardiac tests, lab results, or imaging studies related to your cardiovascular health.
List of Questions
Write down any questions or concerns you'd like to discuss with Dr. Baiju during your consultation.
